Are You a New Patient?
Getting started at Olive Branch is simple: just submit your pre-screening form online—or request one by emailing info@olivebranchpsychlv.com—and send over a copy of your valid ID and insurance card. Our team will review your information within 48 to 72 business hours and give you a call to set up your appointment. Scheduling?
To schedule an appointment, please contact our office by phone or complete our online form. We are open Monday through Friday, from 9:00 a.m. to 4:00 p.m. PST, and closed on weekends. When booking, kindly have the following information ready:
- Valid ID (i.e., driver's license)
- Insurance card
New patients are required to register through our patient portal (Charm Health). An invitation will be sent via email, and all questionnaires and intake forms must be completed at least 24 hours before your initial appointment. Please note that failure to complete these forms in advance will result in your appointment being canceled.
Need to Reschedule?
If you need to cancel or reschedule your appointment, please contact our office. We are open Monday through Friday, from 9:00 a.m. to 4:00 p.m., and closed on weekends. We kindly ask for at least 24 hours’ notice for cancellations or rescheduling. A $50 fee will be charged for late cancellations or same‑day no‑shows.
